In medicine and clinical pharmacology, Ideal Body Weight (IBW) serves as a key physiological baseline. While not a measure of healthy body fat percentage directly, clinicians use IBW to calculate dosages for specific medications (including anesthetics, chemotherapy agents, and emergency therapies) where standard body weight could result in toxicity or under-dosing.
The Medical Formulas Explained
These equations were developed using linear regression based on clinical study groups. Most equations calculate the ideal weight for a base height of 5 feet (60 inches) and add a scale factor for each additional inch.
- Devine Formula (1974): Originally designed to compute clearance values for drugs like theophylline:
Male: IBW = 50.0 kg + 2.3 kg per inch over 5 ft
Female: IBW = 45.5 kg + 2.3 kg per inch over 5 ft - Robinson Formula (1983): Devised by clinical researchers to correct deviations in Devine's female ratios:
Male: IBW = 52.0 kg + 1.9 kg per inch over 5 ft
Female: IBW = 49.0 kg + 1.7 kg per inch over 5 ft - Miller Formula (1983): Developed using independent statistical models for balanced adult averages:

Clinical Limitations & Healthy BMI Ranges
While these formulas are statistically sound, they have limitations because they only factor in gender and height. They do not account for muscle mass (such as in athletes), bone structure, or overall body composition. Therefore, health practitioners also reference the Body Mass Index (BMI) healthy weight range as a broader indicator. A healthy weight corresponds to a BMI between 18.5 and 24.9, which scales proportionally for different body sizes.
